چکیده

A systematic review of randomised controlled trials of the effectiveness of tranexamic acid (TXA), which reduces clot breakdown, in patients with bleeding after severe injury shows that this inexpensive drug could save tens of thousands of lives a year. The review found that TXA reduces the risk of death in injured patients with severe bleeding by about 10% compared with giving no treatment. This would equate to more than 70 000 lives a year if treatment were rolled out world wide (see table 1). On the basis of these new findings, the British military have already started using TXA to treat soldiers wounded in battle in Afghanistan, and TXA is now being incorporated into civilian trauma treatment protocols around the world. Most of the data in the review came from the international CRASH-2 trial, which recruited 20 211 patients with bleeding trauma from some 40 countries world wide and showed that TXA significantly reduces mortality with no apparent increase in the risk of vascular occlusive events. Because the CRASH-2 results are based on large numbers of patients, with both blunt and penetrating trauma, and from many different countries, they can be generalised widely. Given the high quality of evidence for the benefits of this drug, the authors recommend that TXA is considered for use in every patient with severe bleeding from traumatic injury. Studies are now underway to see if TXA can reduce deaths from post partum haemorrhage (which kills about 100 000 women each year, most of whom live in developing countries) and traumatic intracranial bleeding which is a major cause of death and disability. T rauma is a major cause of death world wide. The major cause of trauma-related death is hemorrhage which can also contribute to multiorgan failure-associated death. It is known that antifibrinolytic agents reduce perioperative blood loss and the need of transfusion (1). Because the haemostatic responses to sur gery and trauma seem to be similar, it was su pposed that tranexamic agent, an antif...

Tranexamic Acid – a Recipe for Saving Lives in Traumatic Bleeding

Using kitchen scales, carefully weight out 4 kg of rice and pour it into a deep saucepan. Now put your hands into the rice and let the grains run between your fingers. Contemplate carefully each grain. The number of grains (about 140,000) is approximately the number of lives that could be saved each year world-wide if all hospitalized trauma patients with significant bleeding were treated with ...

Comparing Adrenaline with Tranexamic Acid to Control Acute Endobronchial Bleeding: A Randomized Controlled Trial

Background: Hemoptysis occurs due to either pulmonary diseases or bronchoscopy interventions. The aim of the present study was to compare the efficacy of the endobronchial instillation of adrenaline with that of tranexamic acid. Methods: Fifty patients were randomly selected as 2 double-blinded sample groups (n=25). In these patients, bleeding could not be controlled with cold saline lavage dur...

Role of Tranexamic Acid in Treatment of Patients with Epidural Hematoma

Introduction: Traumatic brain injury is considered as the major cause of mortality and hospitalization of about ten million people across the globe. Most of these patients are young and active people suffering disabilities. Tranexamic acid therapy is commonly used to reduce blood loss and need for blood transfusions in surgical cases. This study aimed at investigating the effect of tr...

Battlefield administration of tranexamic acid by combat troops: a feasibility analysis.

This paper suggests that 1 g tranexamic acid should be incorporated as an intramuscular auto-injector and issued to combat troops for self- or buddy-administration in the event of suffering severe injury. Early administration of tranexamic acid has shown to be beneficial in preventing death from bleeding in trauma patients in both the military and the civilian settings. Tranexamic acid is cheap...
